Rules and Regulations
OFFICIAL RULES
1. SPONSOR: The Sweepstakes is sponsored by Transitions Optical, Inc., 1575 Bishop Street North, Suite 200 Cambridge Ontario N1R 7J4.
2. ELIGIBILITY: The Sweepstakes is open to registered Eye Care Professional locations in Canada including Quebec who have an assigned LIN (Location Identification Number). For those locations without a pre assigned number, requests through contacting Transitions Customer Service Canada (1-877-254-2590) will be provided.
Sweepstakes is not open to employees and directors of the Sponsor or any of their respective parent companies, affiliates, subsidiaries, advertising or promotional agencies, Sweepstakes judges and legal advisors, or to the immediate family members of, or to any persons domiciled with, any such person. All winners are subject to Sponsor's verification of eligibility hereunder.
3. ENTRY PERIOD: The Sweepstakes will begin at 12:01 AM (EST) on May 1, 2009 and end at the conclusion of the RBC Canadian Open on July 26, 2009. The deadline for entry to the program is 11:59 PM on July 22, 2009.
4. ONLINE PARTICIPATION: There is no purchase required for entry to the Sweepstakes. The only way to enter the sweepstakes is to visit www.transitions.com/golfpool - enter through the Eyecare Professional link and then through your location LIN and create a password, complete the Team Entry form by choosing the 5 players which your office believe will finish, in order atop the leader board of the RBC Canadian Open held July 22 - 26, 2009 in Oakville ON at the Glen Abbey Golf Club. Teams are limited to one per email address. The winning team will be the one with the lowest combined score (of their 5 players) as it relates to Par at the conclusion of the tournament. In the case of a tie there will be a random draw conducted from all eligible teams.
5. PRIZES:
All prizes total $10,000.00 in value.
1st Place Prize - will be awarded to the winning team on July 27, 2009 at approximately 9:00 AM EST at Promotion Headquarters in Mississauga Ontario.
Grand Prize includes one (1) trip for 2 to the Transitions Championship for Healthy Sight at the Innisbrook Resort and Golf Club in Florida USA during the PGA Tour event in March, 2010 including on-site accommodations for up to 7 nights, as well as tickets to the tournament for all days - a $7,000.00 value.

Prizes will be awarded instantly, online upon registration of teams throughout the sweepstakes period.
1st, 2nd and 3rd place winners will be contacted by phone, email and/or mail following the draw date.
Winners will be required to claim their prize by completing and signing an affidavit of eligibility, a liability release form, skill testing question and a publicity release. In the event of non compliance with these requirements, an alternate winner will be selected, with no further compensation. If notification of prize award is returned to Sponsor, as undeliverable, the prize will be awarded to an alternate winner, with no further compensation.
Winners may waive the right to receive a prize. The return of any prize or prize notification as undeliverable will result in disqualification, the forfeiture of all interest in the prize, and selection of a substitute winner from among all remaining eligible entries. Unless prohibited by law, acceptance of a prize constitutes permission for the Sponsor and its agencies to use the Winner’s name, address (city and province) and/or likeness for advertising and trade purposes without further compensation, worldwide and in perpetuity, in any and all forms of media, now known and hereafter devised, including without limitation the Internet.
6. GENERAL CONDITIONS: By entering, participants agree to be bound by these Official Rules and by the decisions of Sponsor and Sweepstakes judges appointed by Sponsor, which shall be final and binding in all respects. Sponsor reserves the right in its sole discretion to disqualify any individual it finds to be tampering with the entry process or the operation of the Sweepstakes or web site or to be acting in violation of these Official Rules or in an un-sportsmanlike or disruptive manner. Any person attempting to defraud or in any way tamper with this Sweepstakes will be ineligible for prizes. In the event that any tampering, fraud, virus, bug, technical error or failure, unauthorized intervention, or other problem which corrupts or affects the administration, security, fairness, integrity of the online portion of the Sweepstakes, or for any reason Sponsor deems it necessary, the Sponsor may at its sole discretion cancel, terminate, modify or suspend the online portion of the Sweepstakes and award any remaining prizes at random among all eligible entries received up to the time of impairment. Should the Sweepstakes be cancelled or altered prior to the stated expiration date, notice will be posted at http://www.transitions.com/golfpool.
CAUTION: ANY ATTEMPT TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, THE SPONSOR RESERVES THE RIGHT TO PROSECUTE AND SEEK DAMAGES OR OTHER REMEDIES FROM ANY SUCH PERSON(S) RESPONSIBLE FOR THE ATTEMPT TO THE FULLEST EXTENT PERMITTED BY LAW.
7. DISCLAIMERS: By entering, participants acknowledge and agree that the Sponsor, Transitions Optical Inc., and their respective parents, subsidiaries, affiliates, franchisees, advertising and promotional agencies and the officers, directors, employees, agents and representatives of each of the foregoing companies (collectively, the "Sweepstakes Entities"), will not be liable for losses or injuries of any kind resulting from acceptance/possession/use and/or misuse of prize(s), travel to and from any prize-related activity, participation in the Sweepstakes, printing, mailing or distribution errors, or any malfunctions of the computer Internet system, computer dating mechanism, computer equipment, software, or any combination thereof, unauthorized human intervention in the Sweepstakes, for errors in the administration of the Sweepstakes or the processing of entries, or for any entries that are late (including delayed data transmissions), tampered with, garbled, incomplete, or otherwise not in compliance with these Official Rules. Sweepstakes Entities are not responsible or liable for any injury or damage to any participant's or third person's computer and/or its contents, related to or resulting from Sweepstakes. Participants release and agree to hold harmless Sweepstakes Entities from any and all liability from any of the claims described above.
Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to its web sites. Sponsor is not responsible for any problems or technical malfunction of any telecommunications network or telecommunication lines, computer on-line systems, servers, computer equipment, telecommunications network equipment, software, failure of any e-mail or entry to be received by Sponsor on account of technical problems, human error or traffic congestion on the Internet or on any web site, or any combination thereof.
8. SWEEPSTAKES WINNERS' LIST: For the name and hometown of the Prize Winner's (available after July 27, 2009), and/or a copy of these Official Rules, send a self-addressed, stamped business-size envelope to Transitions ForeEyes Golf Pool Promotion, PO Box 43149 RPO Parkway West, Mississauga, ON L5B 9Z9, Attn: Promotions Department. Indicate whether you are requesting the Winners list or a copy of Official Rules or any combination of the foregoing.
9. In the Province of Quebec any litigation respecting the conduct or organization of a publicity contest may be submitted to the Regie des alcools, des courses et des jeux for a ruling. Any litigation respecting the awarding of a prize may be submitted to the Regie only for the purpose of helping the parties reach a settlement.
